Below is a list of where your Medical Records may be found. This information is up to date, so if your doctor's name is not on this list, than the Essex County Medical Society can not help you. You can contact the College of Physicians and Surgeons of Ontario at www.cpso.on.ca or 1-800 268-7096 ext. 306.

 

Also please note that there is usually a fee charged for making a copy of these records which belong to the Doctor and have to be kept by him/her for a number of years. More information is available for the physician and patient in the PDF file below. This information has been taken with permission from the College website. If looking for records from a specialist, very otten these recports have been sent to your family doctor and can be obtained from him/her.
